| Brand Name: Zocor Generic Name: Simvastatin Drug Class: ANTILIPEMIC AGENTS
INDICATIONS
Adolescent Patients with Heterozygous Familial Hypercholesterolemia (HeFH)
ZOCOR it is designated as addition to a diet to reduce total, LDL-C, and Apo B levels in juvenile boys, and girls who - at least one year post-menarche, 10-17 years of age, with a heterozygous family hypercholesterinemia if after adequate court above a dietetics, during time results are present
:
- LDL cholesterol remains ≥ 160 mg/dL
- LDL cholesterol remains ≥ 190 mg/dL
- Two or more other CVD risk factors are present in the adolescent patient.
- There is a positive family history of premature cardiovascular disease (CVD).
The minimum purpose of processing in pediatric and juvenile patients will be to reach on the average LDL-C <130 mg/dL. Optimal age to initiate therapy depressed a lipid to reduce risk of characteristic computerized projection of adulthood has not been specific.
Patients with Hypercholesterolemia Requiring Modifications of Lipid Profiles
ZOCOR is indicated to:
- Reduce total-C and LDL-C in patients with homozygous familial hypercholesterolemia as an adjunct to other lipid-lowering treatments (e.g., LDL apheresis ) or if such treatments are unavailable.
- Treat patients with primary dysbetalipoproteinemia (Fredrickson type lll hyperlipidemia).
- Treat patients with hypertriglyceridemia (Fredrickson type lV hyperlipidemia ).
- Reduce elevated total-C, LDL-C, Apo B, and TG, and to increase HDL-C in patients with primary hypercholesterolemia ( heterozygous familial and nonfamilial) and mixed dyslipidemia ( Fredrickson types IIa and IIb 4 ).
Reductions in Risk of CHD Mortality and Cardiovascular Events
At patients at high risk of coronary events because of existing coronary cardiac illness, Diabetum, peripheric illness of the pot, the history of a course or other cerebrovascular illness, ZOCOR is designated to
:
- Reduce the need for coronary and non-coronary revascularization procedures.
- Reduce the risk of non-fatal myocardial infarction and stroke.
- Reduce the risk of total mortality by reducing CHD deaths.
In patients with CHD or at high risk of CHD, ZOCOR can be started simultaneously with diet.
Ingredients changing a lipid should be used in addition to a diet circumscribed in saturated fat and a cholesterin.
ADMINISTRATION & DOSAGE
Definitions of a lipid should be performed after 4 weeks of therapy and periodically after that. For patients at high risk for CHD an event, owing to existing coronary cardiac illness, Diabetum, peripheric illness of the pot, a history of a course or other cerebrovascular illness, a recommended launching dose - 40 mg / days. A recommended routine launching dose - 20 - 40 mg once a day in the evening. See below for references of dosage in express bunches (that is, a homozygous family hypercholesterinemia, teenagers and renal failure) or for the patients receiving concomitant therapy (that is, cyclosporine, danazol, amiodarone, Verapamilum, or gemfibrozil).
Range of dosage - 5-80 mg / days. Dosage should be individualized according to the purposes of therapy and patient's reactions. (For reduction of risks of the main coronary events, Clinical researches in Adults. For processing the adult dyslipidemia, see NCEP Guides of Processing.) the patient should be placed in a standard diet depressed a cholesterin. At patients with CHD or at high risk CHD, ZOCOR can be started simultaneously with a diet.
Patients with Renal Insufficiency As caution should be carried out, when ZOCOR is applied to patients with serious renal failure; such patients should be started in 5 mg / days and should close monitored. However, ZOCOR does not tolerate significant renal abjection, paravariation of dosage should not be necessary at patients with moderate to reduce renal failure.
Patients taking Amiodarone or Verapamil At patients accepting amiodarone or Verapamilum together with ZOCOR, the dose should not exceed 20 mg / days.
Patients taking Cyclosporine or Danazol At patients accepting cyclosporine or danazol together with ZOCOR, therapy should begin from 5 mg / days and should not exceed 10 mg / days.
Concomitant Lipid-Lowering Therapy ZOCOR one is effective or when it is used in a combination with sequestrants materials of a bile acid. If ZOCOR it is used in a combination with gemfibrozil, dose ZOCOR should not exceed 10 mg / days.
Adolescents (10-17 years of age) with Heterozygous Familial Hypercholesterolemia Adjustment should be made with interspaces in 4 weeks or more. Doses should be individualized according to the recommended purpose of therapy. A recommended range of dosing - 10-40 mg / days; a maximum, recommended a dose - 40 mg / days. A recommended routine launching dose - 10 mg once a day in the evening.
Patients with Homozygous Familial Hypercholesterolemia Recommended dosage for patients with a homozygous family hypercholesterinemia - ZOCOR 40 mg / days in the evening or 80 mg / days in 3 20 mg separate doses, 20 mg, and an evening dose of 40 mg. ZOCOR it should be used as addition on other processings depressed a lipid (for example, LDL apheresis) in these patients or if such processings are inaccessible. |
